“GIFT OF LIFE” - Rural patient assessment with Commwell platform in India.

Healthcare in India has improved considerably since independence. However the problems are still enormous. Access to primary health care is inadequate to the majority of the people because of low availability of basic preventive health care packages, clinics, doctors, drugs and paramedical persons in rural areas. In order to make rural health care services more effective to the rural community, basic changes in the approach to the entire health care delivery system in general and rural health care in particular is required.

The program called the “GIFT OF LIFE” was setup in cooperation with the India’s Ministry of Health in regions of Haryana, Lucknow and Pune, and enrolled over 170 remote clinics driven by the Commwell’s Health eCare remote patient management platform.

The Setup

The Commwell platform is designed to be easy to use with a very intuitive user interface. The logistics consisted of setting up the system in the clinics and training of local and other personnel and overseeing and helping with patient contact and management. The routine management was simple and the main tasks were the continuous monitoring and the data acquisition and analysis. Following the initial regular visit with the local clinic team the patients were randomly referred to the e-health care booths which were equipped with a set of instruments for the measurement of blood pressure, pulse, weight, temperature, oxygen saturation, blood glucose level and a Commwell Physioglove 12-lead ECG.

All of the measurement data was transmitted to a tablet and was automatically uploaded to to the Commwell eHealth cloud-based remote patient management platform. Additionally, the platform noted all of the patient answers to a set of questions covering their symptoms and complaints as well as a series of questions covering the patient’s past history (major diseases, medications, allergies and hospitalizations), all of which were also recorded in the data. All the information was made accessible to the health team.

Electrocardiography (ECG) The PhysioGlove

The project included, apart from the above-mentioned data, the performance of a routine, standard 12-lead electrocardiogram to all patients by means of the Commwell 12 lead ECG PhysioGlove.

In the initial pilot study in Haryana special telecommunication arrangements were made to allow Haryana nominated cardiologists immediate access to our cloud-based monitoring application for expert analysis. Over 20% of the patients were found to have ECG pathologies that warranted physician attention.

The team there noted that performing this quick and excellent quality ECG testing with non-skilled personnel, following a short training as a significant milestone and one that makes the Physioglove 12 Lead ECG invaluable for millions of patients in the rural populationsץ

Conclusion

The medical problems of this population are somewhat similar to rural populations of many western societies. They did not encounter patients with severe infectious diseases (95% of the pilot patients had normal body temperature) or other medical conditions that afflict less fortunate societies. We have rather encountered a population that suffers from the endemic problems of “modern” societies such as Hypertension, Diabetes, Heart disease etc. Health-eCare enabled non-professional staff to perform above mentioned tasks and more and that typically require professional training and accreditation. The fact that a lay person can perform medical examinations on 50 or more patients per day having had a 1–2-day training is due to the combination of a carefully engineered system that includes unique and patented self-use diagnostic devices, multi-language voice guidance (Including Hindi) and algorithms that track every examination step and provide variable real-time instructions.

The pilot program revealed a tendency of accelerated aging of the population, a problem that is bothering most advanced societies. It is worth noting that the Indian population census shows that more than 60% of the elderly population resides in rural areas which further complicates the problem. This aging trend together with the declining birth rate in India indicates that India will be confronted with a huge problem of the future health and wellness management of its aging population. As mentioned above, these problems are even more complex since most of the aging population lives in rural areas. Thus, the Commwell technology provides an efficient and cost-effective solution to the worldwide aging problem, specifically those in remote locations and therefore seems to be a perfect solution for the Indian rural populations.
